Medical Records
Release of information must comply with State and Federal guidelines.
Information and requests for Medical Records and Information are dependent upon where you received your treatment.
Please contact Health Information at 801-581-2704 if you have questions regarding the release of patient information
To obtain copies of your orthopaedic medical records or MRI's, etc., please print a copy of the Patient Authorization Form (PDF).
Fill out the Patient Authorization for with the following information:
- Patient name, Date of Birth and Social Security Number
- Information being requested (please explain be as detailed with your request as possible) and dates of service
- The name and address of the person the information will be released to
- The signature of the patient, signed within 1 year (Signature must be notarized or witnessed by a University Orthopaedic Center Staff).
- Note: For copies of films onto CD, contact the UOC; for film copies, contact Hospital Medical Records http://uuhsc.utah.edu/hi/records/
You may leave your patient authorization form with the UOC clinic, 2nd floor.
Your request for medical information will be processed in a timely manner. Please allow additional time for delivery.
